
Recognition Programs
Southwest Care Center (SCC) values the hard work and dedication of its employees and recognizes the
importance for team members to feel valued, recognized, and appreciated. The SCC Employee Recognition
Policy and Procedures provide a framework for individual team members to be recognized for their
outstanding contributions.
While continuous informal recognition is encouraged at the team level, the organization formally
recognizes team members for their significant contributions and demonstration of the Southwest Care
Center core values. Southwest Care Center employees have the opportunity to participate in the following recognition programs.
• Top Contributor Program: This program is designed to recognize quarterly those team members who demonstrate a commitment to the organization in a manner that is above and beyond expectations of their position. The quarterly selectee will have developed and implemented a significant operational improvement that results in cost savings or revenue generation for the organization.

Criteria: Team member who demonstrates commitment to the organization’s growth and sustainability by developing and implementing an operational efficiency, cost savings, and/or revenue generating idea. Must be above and beyond of the scope of their assigned job duties but may be related. (Example: Patient Access Specialist develops a workflow improvement that reduces patient wait times by 10%). The idea must be implemented with verifiable outcomes.

Eligibility: All staff other than Executive Leadership Team and Directors who are in good standing and have no current disciplinary actions.

Submissions: Department Directors and Managers can submit via “Top Contributor Submission” link SCC SharePoint Main Page. Director or Manager should work with team member to fully describe the contribution ensuring objective and measurable outcomes are demonstrated in the submission. Only fully completed submission forms received via the link will be considered for selection.

Selection: Top Contributor of the Quarter submissions will be evaluated and selected by the Executive Leadership Team in the month following the end of the quarter.

Awards: There will be one (1) quarterly winner announced who will receive a $1,000 cash prize. Notification will be made via SCC Teams Chat.
• Nacho Average Team Member Program: This program is designed to recognize those team members who exemplify the SCC Core Values. Team members are submitted by their peers with one selectee being awarded employee of the month.

Criteria: Any team member who significantly demonstrates one or more of the SCC Core values and is recognized by a peer via the submission process as outlined below.

Eligibility: All staff, other than Executive Leadership and Directors, may be nominated by their peers for the Nacho Average Team Member of the Month. However, the monthly selectee must be in good standing and have no current disciplinary actions. A team member can only be Nacho Average Team Member (NATM) one (1) time during a calendar year.

Submissions: Any team member can submit a fellow team member via the “Nacho Average Team Member Submission” link on the SCC SharePoint Main Page. All submissions must be complete and describe fully how the team member exemplified the SCC Core Values.

Selection: Nacho Average Team Member of the Month submissions will be evaluated and selected by the Employee Experience Team based through a random drawing from all qualified submissions. Only one entry into the drawing per team member will be allowed regardless of the number of submission made for the individual. The selectee will be announced the following month. There will be one (1) winner monthly.

Awards: Nacho Average Team Member will receive an Amazon Gift Card (total value: $150). Awardees will be notified via SCC Teams Chat. Other nominees will also be listed in recognition of their own contributions.
• Team Member of the Year Program: This program is designed to bring special recognition to those who have been awarded as a selectee under the two programs above with final selectee being awarded SCC Team Member of the Year.

Criteria/Eligibility: Winners of the Top Contributor of the Quarter or Nacho Average Team Member of the Month are eligible for Team Member of the Year. All eligible selectees must still be in good standing with no disciplinary actions to remain eligible for consideration as Team Member of the Year.

Selection/Submissions: The HR Department will maintain the list of eligible selectees. All SCC team members will be able to submit their choice for Team Member of the Year from the list of eligible selectees. Submissions will be made via a “Team Member of the Year” link on the SCC SharePoint Main Page. A final review by the Executive Leadership Team and the Employee Experience Team will occur before final announcement.

Awards: Trip for 2 (up to a total value of $3,000). Team Member of the Year will work with local travel agency or the HR Department. Note that trips cannot be booked until March 1st or forward following notification. This trip value is not exchangeable for cash payment. ETO/PTO is required to be used for any days the employee is scheduled off from work as a result of this trip. Announcement of Team Member of the Year will occur at Annual Holiday Celebration.
Important Notes: Any monetary awards provided as part of these recognition programs are considered taxable income and subject to applicable Federal and State taxes. Additionally, each Recognition Program is subject to termination and/or modification by the Chief Executive Officer without prior notice.
